A company is looking for a Care Navigator specializing in kidney
health management.
Key Responsibilities
Manage case load and coordinate care for patients with kidney
health issues
Educate members on kidney health, dialysis, and transplant
options
Collaborate with healthcare professionals to ensure positive
patient outcomes
Required Qualifications
Current RN license in Michigan
Bachelor's degree in Nursing
Three years of experience in case management preferred
Experience in a dialysis or transplant center preferred
Familiarity with Medicare and Medicaid preferred
